South Weymouth, MA—November 5, 2012—Thanksgiving is a time to gather with family and friends and for great food and great conversation. It is also a great time to “talk turkey” with your loved ones about who will speak for you in the event of a health care emergency.
Beginning Tuesday, November 13, South Shore Hospital colleagues will be available to answer your questions about health care proxies.
Complete your health care proxy and our team will make copies for your proxy and your primary care physician. We will place a copy in your South Shore Hospital medical record if we are your preferred hospital.
The public is invited to pick up or fill out Health Care Proxies on the following dates and times in the Dining Room at South Shore Hospital:
Tuesday, Nov. 13 between 11 a.m.-1 p.m. and 4:30. - 6 p.m.
Wednesday, Nov. 14 between 11 a.m. - 1 p.m. and 4:30 - 6 p.m.
Thursday, Nov. 15 between 11 a.m. - 1 p.m. and 4:30 - 6 p.m.
