South Shore Hospital part of regional group to receive Patrick Administration Prevention and Wellness Trust Fund grant
South Weymouth, MA—February 3, 2014--South Shore Hospital is pleased to be one of the organizations participating in a community health initiative led by Manet Community Health Center. The purpose of the initiative is to forge and coordinate relationships among organizations, communities and health care providers to fight chronic illness, improve health outcomes and reduce health care costs for the people of our region, and are financed through The Patrick Administration’s Prevention and Wellness Trust Fund.
The South Shore region grantee group includes eight entities: Manet Community Health Center; Bay State Community Services, Inc.; City of Quincy (Quincy Health Department); Quincy Medical Center; South Shore Elder Services, Inc.; South Shore Workforce Investment Board; South Shore YMCA, Inc.; Town of Weymouth, and South Shore Hospital. The group will design a plan to address these pressing health needs and then be eligible for up to $1.5 million per year for three years to implement the plan.
The local group’s focus will be prevention of hypertension, substance abuse, tobacco use, and falls in the elderly population.
