More and more people with chronic heart problems can now receive care in their homes, thanks to a team of experts at South Shore Visiting Nurse Association. Experienced cardiac nurses, physical and occupational therapists, home infusion nurses, social workers, and certified home health aides deliver coordinated, specialized care to these patients throughout Southeastern Massachusetts.
Congestive heart failure — when the heart loses its ability to pump enough blood through the body — is the most common diagnosis among South Shore VNA patients, and requires careful oversight to avoid recurring hospitalization. Our team works together with patients to educate them and their families about the illness, administer necessary medications, and help improve quality of life.
South Shore Visiting Nurse Association's team also cares for patients recovering from surgical procedures, including coronary artery bypass, and even heart transplants. Team members collaborate with hospital-based physicians and nurses to assure seamless care for patients, whether they are discharged from South Shore Hospital or hospitals outside the region.
