South Shore Hospital's home care division provides Visiting Nurse, Hospice, and private duty services for residents of Southeastern Massachusetts. Our expert team of nurses and clinicians can assist individuals of all ages by providing nursing case management, rehabilitation, assessment, education, and other support services necessary for optimal care at home.

Our services have been ranked among the nation’s top 25 percent of home health providers by Outcome Concept Systems, a leading post-acute health care information company.

The home care division consists of three major programs: South Shore Visiting Nurse Association, Hospice of the South Shore, and Home & Health Resources.
