South Shore Hospital's home care division provides Visiting Nurse, Hospice, and private duty services in Southeastern Massachusetts. Our expert team of nurses and clinicians can assist individuals of all ages by providing nursing case management, rehabilitation, assessment, education, and other support services necessary for optimal care at home.
 South Shore VNA has been named to the “2012 Home Care Elite Top 500” based on an independent review of our agency’s patient satisfaction, quality metrics, and operational performance. Our VNA now ranks among the top half-percent of 10,000 home care agencies reviewed by Outcome Concepts Systems (OCS), a leading post-acute health information company.
Our home care division is comprised of three agencies:
A Medicare-certified agency providing home care services, including skilled nursing, specialty disease management programs, physical therapy, occupational therapy, speech therapy, home health aid, and medical social work to adults and children at home.
The region's first Medicare-certified hospice program, offering compassionate home care services to support terminally ill patients, and their families.
A private-duty home care program providing homemaker, personal care aide, certified home health aide, childcare aide, and nursing services for individuals of all ages.
