Hospice of the South Shore is able to bring care to you or your loved one in the comfort of your home, an assisted living facility, or long-term care facility. Our multidisciplinary team provides everything from nursing and rehabilitation services for the patient to respite care and bereavement programs for the family. The team includes nurses, rehabilitation therapists, social workers, aides, volunteers, and others.

What to Expect
When a family member or loved one serves as the primary caregiver for the patient, our hospice team provides support and care through scheduled visits. During these visits, our multidisciplinary team evaluates and addresses the needs of the patient and family – with expertise, respect, and a true sense of caring.
Our nurses are available 24 hours a day, seven days a week for emergency consultations and visits. Should hospitalization become necessary, our hospice team continues to support both the patient and family during a related hospital stay.
