Arranging for Services
The Home Care Division of South Shore Hospital accepts patients from a variety of referral sources, including individuals, families, friends, physicians, discharge planners skilled care/assisted/independent living facilities, and community agencies. Although we are able to provide a home evaluation without a physician’s order, we must receive an order from your physician before we can begin providing services in your home.

For Physicians
Your patients are important to you. They are also important to us. With the Home Care Division of South Shore Hospital, you can be assured that the highest level of medical care will follow your patients home.

As your patient’s doctor, you will remain in control of your patient’s care. We will always keep you up-to-date with your patient’s progress and outcomes.

Referrals for home care services are accepted all day, every day through our 24-hour referral line at 1-800-432-9995. If you are making a referral during a discharge from South Shore Hospital, you can simply write “Refer to Home Care Division” in the orders.
When making a referral to the Home Care Division of South Shore Hospital, please supply the following information:
- Patient’s name, address, phone number, date of birth and social security number.
- Physician name, phone number and address.
- Diagnosis
- Treatment or services required
- Medications
- Insurance carrier
