Upon discharge from South Shore Hospital, a patient does not automatically receive a copy of his or her medical record. However, patients have the right to review or receive copies of their medical records upon request.
The fastest and easiest way to obtain a copy of your medical records at South Shore Hospital is to submit a signed and dated Release of Information form. The form must be properly filled out by the patient or patient’s guardian/legal representative and mailed to the Health Information Management Department:

We will mail your records to the address specified on the release of information form. To protect a patient's privacy, we do not fax or email medical records, except in the case of emergency care, to the provider. You also have the option of picking up your health information instead of having it mailed. Photo identification is required.
