Rehabilitation Services

South Shore Visiting Nurse Association (SSVNA) has been caring for patients in the comfort and convenience of their homes since 1911. Our therapists provide care to people along the whole age continuum, from babies to end of life care.

Many patients become deconditioned during an illness or hospital stay and benefit from home therapy to return to their prior activities. Our in-home rehabilitation services include Physical Therapy, Occupational Therapy and Speech Therapy. Last year, rehabilitation therapists made 36,500 visits to patients’ homes.

The Conditions We Treat

We are able to treat a wide variety of diagnoses in the home environment, including the following:

Orthopedic conditions, such as:

  • Arthritis
  • Osteoporosis
  • Total joint replacements
  • Fractures

Neurological conditions, such as:

  • Multiple sclerosis
  • Parkinson’s disease
  • Stroke

Cardiopulmonary conditions, such as:

  • COPD
  • Congestive heart failure
  • Pneumonia.

The Services We Provide

Physical Therapy – Physical Therapists work with patients that are exhibiting a decline in functional mobility due to impaired strength, limited joint mobility, pain and/or neurological impairments.  The physical therapist works with patients in the home to improve strength, range of motion and/or balance to maximize independence in functional mobility.  Education is provided to both patients and care plan partners in regards to equipment need, pain management techniques and exercise programs to maximize functional mobility.

Occupational Therapy – Occupational therapists work with patients that are experiencing difficulty performing the general activities of daily living (ADL’s) due to physical problems that are affecting their functional status.  They educate patients and care plan partners I n exercise programs and equipment to improve function in basic household tasks such as eating, bathing, dressing and also energy conservation techniques.  Occupational Therapists promote maximum independence with functional activities.

Speech Therapy – Speech therapists work with patients that are experiencing difficulties with oral motor control, swallowing, cognition and/or speech and language skills.  Often times, these skills have been affected by a neurological event/disease, head/neck cancer, or due to an underlying medical disease process.  Education is provided to the patients and/or care plan partner in regards to home exercise programs to maximize these skills.
